Preventing the Spread of Disease

1. Immunization Clinics for children, adolescents, and adults. Tetanus, diphtheria, whooping cough, polio, measles, mumps, rubella, haemophilus influenza (HIB), hepatitis A and B, chickenpox, pneumococcal, HPV, meningococcal, rotavirus, influenza, shingles. All vaccines are given according to state, ACIP, and CDC guidelines. Call the Public Health Office for more information or to make an appointment.

2. Infectious Disease Outbreak Investigation and Control  Local Public Health Nurses along with the County Health Officer and the epidemiology team from the Wyoming Department of Health investigate reports of communicable diseases and implement control measures according to state and CDC guidelines.

3. Tuberculosis testing is available to determine whether a person is infected with TB. The tuberculin skin test (TST) is required for health care workers, day care providers, and others. Public Health Nurses can interpret test results and make recommendations for follow-up and treatment if indicated.

4. Public Health Emergency Preparedness and Response. The local Public Health Response Coordinator works with the Wyoming Department of Health, Homeland Security, the County Health Officer, local health care providers, and others in the community to prepare for public health threats like bioterrorism and pandemic influenza.

Promoting Family & Community Health

1. Wyoming Hand In Hand Home Visiting Program provides support and skills to promote healthy pregnancies and strong families. Specially trained nurses visit families regularly during pregnancy and after birth and share information regarding breastfeeding, caring for your baby, baby’s development and communicating with your child.

2. Children’s Special Health provides services to high-risk pregnant women, high-risk newborns, and children with special health care needs from birth to age 19. Covered services include medical evaluations, medical and hospital care, and emergency and urgent travel. Apply at the Public Health Office. 

3. WIC is a service for pregnant, postpartum, or breastfeeding women, and for infants or children under five years old. Nutrition information and food are provided to families that meet financial guidelines. Fathers, guardians, or foster parents may apply for WIC for their children. Increasing Access to Healthcare

1. Screening Assessments (LT-101’s) for Nursing Home Care  Public Health Nurses complete functional assessments (LT-101’s) to determine the appropriate level of care needed by elderly and disabled persons. If nursing home care is not indicated the PHN works with the individual, their family, and community service providers to access needed services. 

2. Home and Community-based Waiver Services are an alternative to nursing home placement for a limited number of Medicaid-eligible individuals who meet nursing home criteria. These persons must be able to live safely in their home environment and choose to do so. Services provided in the home include case management, personal care, respite care for family caregivers, home-delivered meals, prescriptions, personal emergency response system, and non-medical transportation. 

3. The Wyoming Breast and Cervical Cancer Early Detection Program covers the cost of breast exams, pelvic exams, pap tests, mammograms, and some diagnostic testing. Women must be age 50 or older and qualify financially. Call 1 (800) 264-1296.

4. The Wyoming Colorectal Cancer Early Detection Program helps eligible persons with the costs of colonoscopies and polyp removal.
